  • Office Staff welcomes new patients upon arrival for appointments having already printed their online personal and medical history forms for signature. Any special needs or further requirements can be arranged in advance, making office visits less time-consuming for patients and more efficient for staff.
  • Practices begin collecting medical histories in digital format directly from their patients, with full access and ownership of the data. Register, log in, enter data.

Signing up for an account is fast, easy and FREE for patients. Patients establish their account username and password, then enter personal and medical data on a secure server that ensures medical providers maintain HIPAA compliance obligations. For practices where several family members are patients, only one main account is required allowing for entry of unlimited family-member histories and other required forms.

Secure web hosting.

ePatientHistory service is installed on a dedicated HIPAA compliant secure infrastructure. Your account will be issued a unique directory name, and linked to your own website if one exists. You and your staff will have total control of, and access to, your patient data as if it were on your own web host server.

Look and feel.

ePatientHistory webpages and forms are customized to match the "look and feel" of your own website. Graphic elements, navigation to main site pages, and color scheme will be duplicated to seamlessly incorporate online patient data services with existing websites. Our graphic staff will obtain these elements from your existing website, and demands on office staff will be nominal.

Custom forms.

ePatientHistory forms used for online patient data entry are completely customized to each individual practice. This ensures that your patients enter the data required for providing services and care appropriate to your field or specialty, and customization is included in the installation fee. If you want the capability to enter followup data "for your eyes only", this can also be configured. Database contents can be downloaded for importing patient records via CSV files into those EMR systems that support this capability. CSV files are compatible with MS Excel, so data can also be maintained and manipulated using spreadsheets for a variety of uses. Patients may update their personal data, and print medical histories to use for their own purposes. Translation of forms into Spanish or French is available for an additional cost.

HIPAA Compliance.

An agreement will be provided to those practices hosting on our server fulfilling the requirement contained in the implementation of the Health Insurance Portability and Accountability Act ("HIPAA") that each health care provider covered by the act obtain a written contract with other entities that utilize or maintain Protected Health Information ("PHI") obtained from the health care provider.

Simple, functional design.

In order to make data entry effortless for patients, the form design is kept simple and easy to complete, using a combination of text fields and pull-down selection menus.

  • Live Demo. Click to open a sample of ePatientHistory as experienced by a patient. The home page may contain custom information about your practice. Toggle the main titles to view generic information that can be customized to contain details about your practice hours, services and policies.
  • Register. Begin by registering as a patient. You may use your own name, enter a fictious name, or simply log in with username "palford" and password "demo123". (Records you enter in the demo database are periodically deleted.)
  • Add Patient History Once you've registered, log in and choose "Add History" to review a sample form. Feel free to enter data in the fields, but it is not necessary to complete all the fields in the demonstration. You will be prompted for any required fields, which are customized to each practice.
  • Patient History Listing Once a history has been added, click "My History" to review patient history listings. Listings may be edited, reviewed, or deleted. Additional family member listings may be added to the main patient account. The option to allow upload of patient photos is available for those practices where this feature may be appropriate. This option is removed if inappropriate.
  • Select a New Form The default form is "new patient registration", but additional forms may be selected and added to a patient's history listings. Categories appropriate to your practice are configured to allow staff to quickly locate patient records.
  • Patient Account Profile Click "Account" for the main control panel, offering access to patient profile, history listings, and form options. The account profile may be used to gain additional information about your patient, such as emergency contact, living will directives, etc. This data can be updated by the patient as required. Profile form fields are completely customized to fulfill the needs of each individual practice.

Accessing patient data records could not be easier for staff! We've included "help boxes" that contain tips for administering records to get started. Help boxes can be disabled once staff is familiar with the simple admin panel.

We feel it is fair to inform our clients that, as demand for this new service grows and costs for online security increases, this price may change. This fee includes development of up to (4) custom forms, configuration to match the "look and feel" of your existing website, protection by our SSL certificate on a HIPAA-compliant server. A hosting fee of $39.95 per month is applicable and payable one year in advance which includes secure database backups and a 100% restoration guarantee. A custom URL address is established for direct access or linking from an existing website. For example, www.epatienthistory.com/yourdomainname or www.epatienthistory.com/yourpracticename.

Additional forms.

The one-time installation fee includes configuration of up to (4) custom forms. Add $100 for each additional form that may be required. Most practices do not require more than (4) forms. The patient registration form can be configured to contain all pertinent fields in one record, providing a higher level of convenience for patients and staff.

Requirements

Our experience in web design and development is vast. This means the demands on your staff will be minimal. We require printed or digital copies of the forms you wish incorporated, and information about your practice to be included. Having a website is NOT required for offering our services to your patients.

Delivery Time

Once ePatientHistory service is ordered and payment received, normal launch time for review and approval takes one to two weeks, depending on our current development schedule. You and your staff will have an opportunity to test drive, and make any necessary changes before links from your website (or direct patient access links) are made active. Once the configuration is completely approved, your new ePatientHistory service will go "live" immediately.
